Mentor Resources: Online Mentor Meeting Report Form

It is recommended this report be filled out after every meeting with your mentee. This report is due every Friday before 5:00 p.m. This form is time dated to our servers when submitted. Write down a brief synopsis of your meeting with your mentee. What questions did they have? What did you help them with? What did you talk about? Did you notice any unusual behavior? Have there been any progress with goals you set for your mentee? What does your mentee need improvement with? What could you do better at your next meeting? You should submit up to 1 report minimum per mentee.
