How to Create a Unique Code
A unique code is a six digit code that will help to identify your responses to surveys and other work on which you may be asked to provide information of a sensitive or personal nature.
This information will help researchers be able to compare both individual and group data that can be used to assess trends and changes as a result of your participation in this grant program.
The process is simple.
First, write the initial of your mother's maiden name. For example if it is "Alanis", then write an "A".
Second, write your birth order using two digits. For example if you are the second born, write "02".
Third, use "1" if you are a male and "2" if you are a female.
Fourth, write your date of birth using two digits. For example if you were born on the "4th", then write "04".
Thus if you are a female your unique code is "A02204".
