Application Requirements (This items must be included in your packet you turn in)

1. Completed University Scorpion Scholar Application.

2. FAFSA – Copy of the conformation page of completed Free Application for Federal Student Aid or SAR report. (http://www.fafsa.ed.gov )

3. Resume – Create a Resume that list all your extracurricular activities, positions of leadership, honors, awards and community service you have participated in. Make sure to include the length of time held for positions or memberships, dates of awards and honors, and other information to show the timeline when each item began and ended. Community Services and events should also include number of volunteers hours for event. Resume must be similar to sample resume attached.

4. High School Transcript – Please include a copy of your high school transcript (7th semester). Applications without them will not receive consideration. Meet with your high school counselor or records department for copies. You should have two copies, one for the scholarship application and a separate copy must be sent to the UTB/TSC Admissions department.

5. Letter of Intent - Please provide a carefully considered statement that can include any of the following, 1) your academic and professional objectives, 2) explain how UTB/TSC will help you attain your goals, 3) Accomplishments and awards, 4)Leadership Positions, 5) Community Service, 6) Other. Please explain fully. Remember that your letter will be your opportunity to share with the committee why you should be selected over other students. This statement should be no more than 500 words, must be typed, double spaced, 12 pt Times New Roman Font, and include 1 inch margins. Letter should not include any errors in grammar or spelling.

6. Letter of Recommendation – Please submit a minimum of two letters of recommendation from a high school teacher, mentor, or counselor. The letter or recommendation should be written by an individual who is familiar with the student’s academic progress, work and ability to excel in a college environment. Maximum of five letters of recommendation will be accepted.
